Insurance

Information regarding in-network and out-of-network insurance billing is provided below. My office can bill any insurance plan and services will be covered according to what your plan provides.

Aetna (In-network)

If you've got an Aetna insurance plan, your services would be covered at your plan's in-network rate. Since coverage varies from plan to plan, please consider contacting Aetna by visiting this link or calling the number on your insurance card.

Regence Blue Cross Blue Shield of Oregon (In-network)

I am now in-network with Regence Blue Cross Blue Shield and can provide services at your plan's in-network rate. Please contact BCBS directly to learn the details of your plan's coverage.

Cigna (In-network)

I am an in-network provider with Cigna, as of 1/30/2014. I can provide services at your plan's in-network rate. If you're unsure of what your plan includes, I recommend giving them a call to inquire about your plan's mental health coverage. Here is a FAQ Section from the Cigna Behavioral Health webpage. You can find the appropriate contact phone number on the back of your insurance card.

First Health Choice (In-Network)

I am in-network with First Health Choice. Please call the number on your insurance card to get information about your plan's coverage.

LifeWise (In-network)

I am also an "in-network" or a "preferred provider" for LifeWise. This means that services would be covered according to your plan's in-network rate. Each plan is a little different. To find out the specifics of what your plan covers, please contact LifeWise at 800-596-3440 or dial the number on your insurance card.

Medicare (In-network)

I am able to provide services to individuals covered by medicare. Please contact me if you have medicare coverage and you would like to consider receiving services.

Moda (In-network)

Formerly known as ODS, Moda is another plan that I am in-network with. It's important to call the number on your insurance card to find out what your plan covers.

Oregon's Health Co-op (In-network)

I am in-network with Oregon's Health Co-op, so if you are getting your insurance through them, services would be covered consistent with them plan's in-network rate. Please call the number on your insurance card to determine what coverage is included.

PacificSource (In-network)

PacificSource is a great company with which to be a preferred or "in network" provider. Services will be covered according to your plan's in-network rate. Each plan is a little different, so please contact PacificSource at (800) 624-6052 or dial the number on your insurance card.

Providence/PBH (In-network)

Providence uses United Behavioral Health for its behavioral health network. I am in-network with Providence Behavioral Health as well. I encourage you to call the number on your insurance card to determine what coverage for mental health you have.

United/UBH (In-network)

I am in-network with United, which includes a number of other insurance providers. Each plan is different, in terms of what services are covered, so please contact UBH by calling the number on your insurance card to inquire about your plans coverage of mental health.

Out of Network

If your insurance plan is not listed above, don't worry! My office can still bill your insurance company. Many plans will still cover services at your plan's "out of network" rate. If you have any questions about how this might work, please contact me. It will also be useful to contact your provider to see what percentage of fees they will be able to cover for an out of network provider.
